Enrollment Instructions
- How to Enroll:
- Click Benefits.
- Click Members.
- Click Benefit Trust Fund.
- Scroll to the bottom of the page.
- Find Benefit Forms (in red).
- Open the drop-down menu and select benefit trust fund enrollment card for enrollment in Delta Dental PPO and Davis Vision.
- Open the drop-down menu under Delta Dental DHMO Enrollment application to enroll in Delta Care USA (DHMO) instead of the PPO.
