Before Your First Day:
Complete Your Onboarding Forms
Before your first day, complete the required onboarding forms so you can be officially hired and placed on payroll.
Your onboarding forms were sent to the personal email address you provided when you accepted your offer. Please check your inbox and spam/junk folder for an email from Human Resources.
Required Forms:
- Employee Demographic Data Form
- Form I-9 – Employment Eligibility Verification
- Form W-4 – Federal Tax Withholding
- Form IT-2104 – NYS Employee’s Withholding Allowance Certificate
- Direct Deposit Form (AC 2772)

Need to complete your forms in person?
If you are unable to print or complete the paperwork electronically, we'll have everything available for you to fill out during your onboarding session with HR.
Completing Your I-9
On your first day, bring your required identification and work authorization documents to the HR office to complete your Form I-9 in person.
Important: Federal law requires Form I-9 to be completed on or before your first day of work. Please review the I-9 form and instructions in your onboarding packet before you arrive.

Parking on Campus
Day 1 - Get a Temporary Parking Pass
Before parking on campus your first day, stop by the University Police Department to pick up a complimentary temporary parking pass. Old Westbury Visitor Info
Day 2 and Beyond- Purchasing a Parking Permit
All employees must purchase a parking permit beginning on their second day of work.
- Costs $16
- Valid: September 1 through August 31.

Parking Rules to Know:
- One permit per vehicle: Each vehicle you drive on campus must have its own permit. If you drive more than one vehicle, each vehicle must be registered.
- Park only in designated spaces: Parking outside designated spaces may result in a ticket.
- License plate enforcement: University Police use license plate scanning to enforce parking regulations. Unregistered vehicles may be ticketed automatically.
