SUNY has announced a system-wide policy that all students who access campus facilities in person must be vaccinated against COVID-19, with limited exceptions. This requirement went into final effect following the approval granted by the U.S. Food and Drug Administration to the Pfizer vaccine series in late August.
The State University of New York system requires all students engaged in in-person learning or having any on-campus presence to receive a COVID-19 vaccination. Please submit Proof of Vaccination to the Student Health Center by emailing studenthealth@oldwestbury.edu.
Students can also submit an application for either a medical or religious exemption to the Covid-19 vaccine. Submission of a religious or medical exemption is not a guarantee of approval. A decision will be sent via student's Old Westbury email.
Failure to comply with the COVID vaccine mandate will result in administrative withdrawal from classes (i.e. de-registered) in accordance with pre-existing withdrawal and financial liability policies.
Students shall submit proof of vaccination against COVID-19, or the information needed to request a medical or religious waiver from this vaccine requirement, to the SUNY Old Westbury Student Health Center. Methods of submission include:

Other requirements related to health and safety (i.e. masking, social distancing, return to campus requirements, etc.) are expected also for those in this position and those granted a medical and/or religious exemption.
Proof of Covid-19 Vaccination
Proof of vaccination against COVID-19 is defined as the following:

Copy of a vaccine record indicating receipt of all doses necessary to complete an FDA approved, or World Health Organization approved, SARS-CoV-2 vaccine series.

Each page of the submitted documents must have student’s first name and last name, and their date of birth or 700#.

The submitted record must include the specific name of the vaccine(s) given as shown on the first or second column of the World Health Organization’s “Status of COVID-19 Vaccines within WHO EUL/PQ evaluation process” document and the date(s) that any vaccine was given.

Exemptions from the Requirement
Exemptions from the COVID-19 vaccination requirement are allowed only for medical reasons and for religious reasons. Students who are granted an exemption from the COVID-19 vaccine requirement are subject to special obligations to enable them to remain on-campus, which include:

Wearing of masks in all public spaces, maintaining six feet of social distancing where ever practicable, and other posted College requirements.
